This is an uncommon tumour, but it is the most common retroperitoneal sarcoma. It affects adults and is lipomatous with a soft tissue component.
CT

•  Fat components easily identified on CT (HU <-10)—may be slightly higher density than ‘normal’ fat.
•  May be cystic.
•  Differentiation from a benign lipoma is not always possible—however, septations >2 mm thick or nodules suggest sarcoma.
• Mass enhances with contrast.

MRI

•  High signal on T1 and T2 due to fat content.
•  Myxoid type is hyperintense on T2 and shows delayed enhancement post-contrast.
