Very common, affects about 50% of those aged >50 years. Large cysts may cause pain, urinary obstruction, haematuria or hypertension. Often multiple and bilateral. Most cysts are benign. Risk of malignancy can be assessed with the Bosniak classification. Bosniak 2f cysts are followed up; the risk of malignancy is approximately 5%.

• Non-enhancing, well defined
• May be of water density or commonly higher due to proteinaceous contents
